| dc.description | Introdução: O tratamento dos aneurismas da artéria comunicante posterior pode ser realizado por via microcirúrgica ou endovascular, mas a capacidade de realizar uma decisão baseada na evidência sobre qual a melhor opção terapêutica tem ainda muitas limitações. Neste estudo, pretendemos avaliar potenciais vieses de seleção que possam influenciar a nossa capacidade de tirar conclusões sobre a eficácia e a efetividade comparativas das duas formas de tratamento de aneurismas. Objetivo: Avaliar a presença de potenciais características do paciente / aneurisma que constituam vieses na opção pelo tratamento endovascular ou neurocirúrgico dos aneurismas da artéria comunicante posterior. Métodos: Neste estudo de coorte retrospectivo unicêntrico, foram incluídos todos os doentes com um aneurisma da artéria comunicante posterior tratados por clipagem neurocirúrgica ou embolização endovascular entre janeiro de 2010 e janeiro de 2021. Os dados clínicos e morfológicos foram colhidos dos registos clínicos eletrónicos para análise estatística. Resultados: 64 doentes foram incluídos neste estudo. 24 (37,5%) doentes foram propostos para tratamento neurocirúrgico e 40 (62,5%) para tratamento endovascular. 10 pacientes (25%) cruzaram para o grupo de clipagem, enquanto nenhum cruzou para o grupo endovascular. A análise baseada no tratamento efetivo mostrou diâmetros significativamente maiores do vaso-mãe (teste-T, p = 0,034) e do colo do aneurisma (Mann-Whitney, p = 0,029) no grupo de clipagem e maiores aspect e dome-to-neck ratios no grupo de embolização endovascular (Mann-Whitney, p = 0,008). Foi encontrada uma frequência de vasospasmo significativamente maior no grupo de clipagem, mas apenas na análise por intenção de tratar (Qui-quadrado, p = 0,032). Conclusão: As diferenças morfológicas significativas encontradas entre os grupos endovascular e cirúrgico na análise de tratamento efetivo e as diferenças na análise por intenção de tratar podem corresponder a uma limitação na comparação direta entre as duas opções de tratamento e sugerir a presença de um possível viés de seleção. | |
| dc.description.abstract | Background: Microsurgical clipping and endovascular coiling are viable treatment options for posterior communicating artery (PComA) aneurysms, but there are still major limitations to evidence-based decisions regarding standard-of-care treatment. In this study, we aimed at assessing potential selection biases that may influence our ability to extract conclusions about comparative effectiveness or efficacy of the aneurysm treatment. Objective: To study the patient/aneurysm characteristics as possible biases in the option for endovascular or neurosurgical treatment of PComA aneurysms. Methods: A single-center, retrospective cohort study was performed, including all patients with treated PComA aneurysms with neurosurgical clipping or endovascular coiling between January of 2010 and January of 2021. Clinical and morphological data were collected from electronic records and statistical analysis was performed. Results: A total of 64 patients were eligible for inclusion. 24 (37,5%) patients were proposed for neurosurgical treatment, while 40 (62,5%) for endovascular treatment. 10 patients (25%) crossed-over to the clipping group whereas none crossed-over to the coiling side. Actual treatment analysis showed significantly higher diameters of mother-vessel (T-test, p=0,034) and aneurysm neck (Mann-Whitney, p=0,029) in the clipping group and higher aspect and dome-to-neck ratios in the endovascular group (Mann-Whitney, p=0,008). A significantly higher vasospasm frequency was found in the clipping group but only in the intention-to-treat analysis (Chi-Square, p=0,032). Conclusion: Significant morphological differences between effective endovascular and surgical groups and differences on intention-to-treat analysis may limit the validity of a direct comparison between treatment options and suggest the presence of a possible selection bias. | |
