Title: Sodium-Glucose Cotransporter 2 Inhibitors in Heart Failure - a review

Abstract: Sodium-glucose co-transporter 2 inhibitors are a class of drugs that have recently been studied for their potential benefits in heart failure. The purpose of this review is to expose, analyze and discuss the existing evidence on this topic. For this literature review, eight articles, selected on PubMed, corresponding to cardiovascular outcomes clinical trials where the use of sodium-glucose co-transporter inhibitors was studied in both patients diagnosed with type 2 Diabetes Mellitus and cardiovascular disease, cardiovascular risk factors, and patients diagnosed with heart failure. Subsequently, these were individually analyzed regarding the study population, intervention and outcomes. One article regarding the use of Empagliflozin on acute decompensated heart failure was also included. The main adverse effects of each study were also discussed. Within the eight clinical trials, a total of 59 747 patients were analyzed. The reduction of hospitalizations for heart failure was constant in all clinical trials, in patients with and without type 2 Diabetes Mellitus. Other parameters such as reduced cardiovascular mortality were also reported in several of the clinical trials. This pharmacological class was shown to be quite safe, and severe adverse events were rare. Inhibitors of the sodium-glucose co-transporter 2 proved to be drugs with important potential in preventing heart failure and with a favorable safety profile.
Description: Os inibidores do co transportador de sódio-glicose 2 são uma classe de fármacos que começou recentemente a ser estudada pelos seus potenciais benefícios na insuficiência cardíaca. O objetivo desta revisão é expor, analisar e discutir a evidência existente neste tema. Para esta revisão de literatura foram selecionados, na PubMed, oito artigos correspondentes aos ensaios clínicos de estudo dos outcomes cardiovasculares na terapêutica com inibidores do co transportador sódio-glicose 2 tanto em pacientes com o diagnóstico de Diabetes Mellitus tipo 2 e doença cardiovascular e/ou fatores de risco cardiovascular, como em pacientes com o diagnóstico de insuficiência cardíaca. Posteriormente estes foram, individualmente, expostos no que diz respeito à população em estudo, intervenção e outcomes. Foi também incluído um artigo referente à utilização de Empagliflozina na insuficiência aguda descompensada. Os principais efeitos adversos de cada estudo foram também apresentados. Dentro dos oito ensaios clínicos, um total de 59 747 pacientes foram analisados. A redução de hospitalizações por insuficiência cardíaca foi uma constante em todos os ensaios clínicos, em pacientes com e sem Diabetes Mellitus tipo 2. Outros parâmetros como redução da mortalidade cardiovascular também foram reportados em vários dos ensaios clínicos. Esta classe farmacológica mostrou ser bastante segura, sendo os eventos adversos raros. Os inibidores do co transportador sódio-glicose 2 provaram ser fármacos com importante potencial na prevenção de insuficiência cardíaca e com um perfil de segurança favorável.
