dc.descriptionO acidente vascular cerebral (AVC) é a principal causa de morbilidade e mortalidade nos países industrializados, incluindo Portugal. A anemia é frequentemente observada como uma condição pré-mórbida dos doentes com AVC isquémico, apresentando uma efeito negativo no prognóstico dos doentes com AVC isquémico tratados conservadoramente. Relativamente ao seu efeito em doentes tratados com terapêuticas de reperfusão, cujo objetivo é recuperação de tecido cerebral em isquemia, ainda permanece fracamente estudada. Assim, pretendemos verificar o efeito que a anemia pré-procedimento de reperfusão, assim como o desenvolvimento de anemia precoce pós-procedimento terá no prognóstico de doentes com AVC isquémico submetidos a tratamento de reperfusão. Para testar a hipótese proposta realizámos uma análise retrospetiva dos doentes com AVC isquémico submetidos a tratamentos de reperfusão (trombectomia e/ou trombólise) no ano de 2019 admitidos no Centro Hospitalar de São João (CHSJ) (incluindo os doentes submetidos a trombectomia no Centro Hospitalar Universitário do Porto e posteriormente transferidos). A presença de anemia (pré e pós tratamento) apresentou uma relação estatisticamente significativa com o desenvolvimento de um pior prognóstico funcional e uma maior mortalidade e esta relação verificou-se também na análise individualizada para cada subgrupo de tratamento (embora apenas tenha sido estatisticamente significativa no caso do prognóstico funcional no grupo submetido a trombectomia e na mortalidade no grupo submetido a trombólise).
dc.description.abstractStrokes are one of the most important causes of mortality and mobility in the industrialized countries, including Portugal. Anemia is a common pre-morbid condition in ischemic stroke patients, and it has a negative impact on the prognosis of patients treated with a conservative approach. Its effect on patients subjected to reperfusion therapy, which aims at rescuing ischemic cerebral tissue, still requires further study. Thus, we planned to study the effect of both pre-procedure and early onset post-procedure anemia on the prognosis of ischemic stroke patients subjected to reperfusion therapy. To test our hypothesis, we conducted a retrospective case-control analysis of ischemic stroke patients subjected to reperfusion therapy (thrombectomy or/and thrombolysis) in the year of 2019 in Centro Hospitalar de São João (CHSJ) - including patients who were subjected to thrombectomy in Centro Hospitalar Universitário do Porto and who were posteriorly transferred. We found the presence anemia (before treatment and after treatment) to have a statistically significant association with the development of worse functional outcomes and mortality rates, and this relationship was also present in every treatment subgroup analysis - though only statistically significant for the functional outcome in the thrombectomy subgroup and for the mortality rate in the thrombolysis subgroup.
