| Title: | Risk factors for gastric neuroendocrine tumors |

| Abstract: | Background: Gastric neuroendocrine tumors (NETs) are a rare type of neoplasm defined as well-differentiated gastric neuroendocrine neoplasms (NENs), which are epithelial neoplasms with neuroendocrine differentiation located in the stomach mucosa. This theme has become increasingly important, as its incidence has increased in recent years and little is known about its etiology. Aim: Evaluate risk factors present in the different types of gastric neuroendocrine tumors based on data from a Portuguese tertiary hospital. Methods: We retrospectively collected data from 67 patients with suspected gastric neuroendocrine pathology diagnosed between 2008 and 2020 in a tertiary hospital (Hospital São João, Porto, Portugal). We compared the 3 types of gastric NET for possible risk factors found in our database and described in the literature. For this comparison, we applied one-way ANOVA test and Pearson's Chi-square test with Fisher's exact test for continuous and categorical variables, respectively. It was considered significant when p <0.05. Results: Significant differences between groups were observed for the factors age at diagnosis, atrophic gastritis and gastric polyps. For the first, there was a higher mean age of type III in our sample compared to type I, and in the other two, the most significant difference was for the absence of atrophic gastritis and gastric polyps in type III NETs. Conclusion: We consider that regarding gastric neuroendocrine tumors we still have a lot to understand, and the etiopathogenesis still needs to be clarified. |
| Description: | Contexto Os tumores neuroendócrinos gástricos (TNEs) são um tipo raro de neoplasia definidos como neoplasias neuroendócrinas gástricas bem diferenciadas (NENs), que são neoplasias epiteliais com diferenciação neuroendócrina localizadas na mucosa do estômago. Este tema tem apresentado uma crescente importância, pois a sua incidência tem aumentado nos últimos anos e ainda se sabe pouco a respeito de sua etiologia. Objetivo Avaliar fatores de risco presentes nos diferentes tipos de tumores neuroendócrinos gástricos tendo como base registos de um hospital terciário em Portugal. Métodos Foram colhidos, retrospectivamente, os dados de 67 doentes com suspeita de patologia neuroendócrina gástrica diagnosticada entre 2008 e 2020 num hospital terciário (Hospital São João, Porto, Portugal), a maior amostra deste tipo conhecida em Portugal. Comparámos entre os 3 tipos de NET gástrico para possíveis fatores de risco encontrados na nossa base de dados e referidos na literatura. Para esta comparação aplicámos o teste one-way ANOVA e o teste do Chi-quadrado de Pearson com teste exato de Fisher para as variáveis contínuas e categóricas, respectivamente. Resultados Observou-se diferença significative entre grupos para os fatores "idade ao diagnóstico", "gastrite atrófica" e "pólipos gástricos". Para a primeira verificou-se na nossa amostra uma idade média superior do tipo III relativamente ao tipo I. Já nos outros dois fatores a diferença mais significativa esteve para a ausência de gastrite atrófica e pólipos gástricos nos TNEs de tipo III. Conclusão Consideramos que em relação aos tumores neuroendócrinos gástricos ainda temos muito que entender e na etiopatogenia ainda há muito por clarificar. |
