| Title: | Early postoperative bleeding after Laparoscopic Roux-En-Y Gastric Bypass: a single center analysis |

| Abstract: | Background: Early postoperative bleeding is a common complication after laparoscopic Roux-En-Y Gastric Bypass (LRYGB) and is associated with significant morbidity. We aimed to identify predictors of early postoperative bleeding after LRYGB and characterize hemorrhagic events and 30-day postoperative outcomes. Methods: We conducted a retrospective cohort study regarding all patients submitted to LRYGB in 2019 at a high-volume obesity center. Early postoperative bleeding was defined as any clinically significant evidence of hemorrhage in the early postoperative period. Demographic, preoperative, and intraoperative factors were evaluated for associations with postoperative bleeding. Postoperative outcomes were compared between patients with and without hemorrhage. Results: Of 340 patients submitted to LRYGB, 14 (4.1%) had early postoperative bleeding. Patients with bleeding had an increased preoperative left hepatic lobe diameter (8.4 vs. 7.3 cm, p=0.048). Prior cholecystectomy (28.6 vs. 14.5%) and previous bariatric surgery (35.7 vs. 23.9%) tended to be more prevalent among these patients. Bleeding occurred at a median time of 31.2 [IQR 19.7-38.5] hours. Thirteen patients presented with intraluminal bleeding and one with extraluminal bleeding. Melena was the most common symptom. All hemorrhages were clinically diagnosed, and 92.9% were managed conservatively. Postoperative bleeding was associated with longer hospital stay (3.5 vs. 2.0 days), higher reintervention (7.1 vs. 0%), and readmission (14.3 vs. 0%), all p<0.05. Conclusions: Bleeding was the most frequent early complication after LRYGB. Patients with hepatomegaly and prior surgeries may have technically challenging LRYGB and should be carefully assessed. Perioperative strategies should be encouraged in high-risk patients to prevent bleeding. |
| Description: | Introdução: A hemorragia é uma complicação comum no período pós-operatório precoce de Bypass Gástrico em Y de Roux por via laparoscópica (LRYGB) e associa-se a uma morbilidade significativa. O objetivo deste estudo é identificar fatores preditores da hemorragia no período pós-operatório precoce de LRYGB e caracterizar os eventos hemorrágicos e outcomes nos primeiros 30 dias após a cirurgia. Métodos: Este estudo de coorte retrospetivo incluiu todos os doentes submetidos a LRYGB em 2019 num centro de elevado volume dedicado à obesidade. A hemorragia pós-operatória precoce foi definida como qualquer evidência clinicamente significativa de hemorragia no período pós-operatório precoce. Fatores demográficos, pré-operatórios e intraoperatórios foram avaliados para potenciais associações com a hemorragia pós-operatória. Os outcomes pós-operatórios foram comparados entre doentes com e sem hemorragia. Resultados: De 340 doentes submetidos a LRYGB, 14 (4.1%) apresentaram hemorragia pós-operatória precoce. Os doentes com hemorragia tinham um diâmetro pré-operatório do lobo esquerdo do fígado superior (8.4 vs. 7.3 cm, p=0.048). A percentagem de doentes que tinham realizado colecistectomia prévia (28.6 vs. 14.5%) e cirurgia bariátrica prévia (35.7 vs. 23.9%) tendeu a ser superior nos doentes com hemorragia. A hemorragia ocorreu num tempo mediano de 31.2 [IQR 19.7-38.5] horas. Treze doentes apresentaram hemorragia intraluminal e um apresentou hemorragia extraluminal. As melenas foram o sintoma mais comum à apresentação. Todos os eventos hemorrágicos foram diagnosticados clinicamente e 92.9% foram tratados de forma conservadora. A hemorragia pós-operatória associou-se a um tempo de internamento hospitalar mais longo (3.5 vs. 2.0 dias), a uma maior percentagem de reintervenção (7.1 vs. 0%) e de readmissão (14.3 vs. 0%), todos p<0.05. Conclusão: A hemorragia foi a complicação precoce mais frequente após LRYGB. A hepatomegalia e as cirurgias prévias podem constituir dificuldades técnicas à realização de LRYGB, pelo que os doentes com estas características devem ser cuidadosamente avaliados. Em pacientes de alto risco devem ser instituídas estratégias perioperatórias para prevenir a hemorragia. |
