| Title: | The Impact of Perioperative Chemotherapy on Survival in Patients With Gastric Signet Ring Cell Adenocarcinoma |

| Abstract: | Background & Aims: Gastric cancer (GC) has a high incidence and high mortality rate. Currently, the standard of care in Europe for locally advanced GC is perioperative chemotherapy followed by surgical resection. However, not all types of GC respond equally to chemotherapy. Signet ring cell carcinoma (SRCC) shows higher rates of chemoresistance and its prognosis remains controversial. We aim to evaluate SRCC's response to perioperative chemotherapy and its impact on survival. Materials and Methods: We conducted a Unicenter transversal retrospective study including all patients submitted to perioperative chemotherapy followed by surgical resection between January 2010 and June 2020. Results: A total of 102 patients with a mean age (SD) of 62,8 (10,2) years were analyzed. No difference was found in the pre-chemotherapy tumor stage and histopathological response in both groups. SRCC showed higher rates of ganglionar invasion (median of 15,5 lymph nodes invaded vs. 3,5 lymph nodes invaded), lower rates of ganglionar regression (median of 33% vs. 46%), and higher rates of chemotherapy adverse effects. Mortality rates were higher in the SRCC group (42,2% vs 30,8%), but this presented no statistical significance. Conclusion: SRCC patients seemed to benefit slightly less from perioperative chemotherapy, therefore, we need to search for other treatment options for these patients. |
| Description: | Introdução e Objetivos: O cancro gástrico tem uma alta incidência e alta taxa de mortalidade. Atualmente, o tratamento standard para tumores gástricos localmente avançados é quimioterapia peri-operatória e recessão cirúrgica. Contudo, nem todos os tipos de cancro gástrico respondem da mesma forma à quimioterapia e esta pode levar a fibrose e edema dos tecidos e atrasar a cirurgia. O carcinoma de células em anel de sinete geralmente mostra altas taxas de quimioresistência e o seu prognóstico é controverso. O nosso objetivo é avaliar a resposta de tumores de células em anel de sinete à quimioterapia e o seu impacto na sobrevivência. Métodos: Realizamos um estudo transversal e retrospetivo, em apenas um centro hospitalar, em todos os doentes submetidos a quimioterapia seguida de cirurgia entre Janeiro de 2010 e Junho de 2020. Resultados: Analisamos um total de 102 doentes com idade média (DP) de 62,8 (10,2). 55,4% dos doentes eram do sexo masculino. 53,6% tinham carcinoma de células em anel de sinete. O estadio do tumor pré-quimioterapia e a resposta histológica do tumor não teve diferenças estatisticamente significativas nos dois grupos. O Carcinoma de células em anel de sinete mostrou taxas mais altas de invasão ganglionar (média de 15,5 gânglios invadidos vs. 3,5 gânglios invadidos), taxas mais baixas de regressão ganglionar (média de 33% vs. 46%) e taxas mais altas de efeitos adversos da quimioterapia. A taxa de mortalidade foi mais alta no grupo de células em anel de sinete (42,2% vs. 30,8%), mas esta diferença não foi estatisticamente significante. Conclusão: Doentes com células em anel de sinete parecem beneficiar menos do tratamento com quimioterapia neoadjuvante, por isso, devemos pesquisar novas opções terapêuticas para estes doentes. |
