| Title: | Progressive microcephaly, spasticity and development delay: Novel SLC1A4 variants in two Portuguese families and literature review |

| Abstract: | Abstract (English version): The role of SLC1A4 gene on the L-serine transport from astrocytes to neurons (via Na+ dependent transporter system ASC) is already an established physiological mechanism. Disease-causing variants in the SLC1A4 gene have been identified as a very rare cause of neurodevelopmental disorder (spastic tetraplegia, thin corpus callosum, and progressive microcephaly - SPATCCM, OMIM 616657), mostly reported in patients with an Ashkenazi-Jewish background (homozygous for the p.(Glu256Lys) variant) (n=15 of the 17 described cases). We hereby report two Portuguese cases presenting with significant global developmental delay, severe progressive microcephaly, spasticity and no Ashkenazi-Jewish ancestry. Whole exome sequencing identified three novel SLC1A4 variants as the likely cause of the disease. Patient 1 is heterozygous for two splice-site pathogenic variants, c.527+1G>T and c.1035-1G>T. The impact on splicing was confirmed by a minigene strategy. Patient 2 is homozygous for a probably deleterious missense variant, c.272T>C (p.(Leu91Pro)), classified as of uncertain clinical significance. Our results support the deleterious role of SLC1A4 variants and highlight the need of considering this entity, regardless of ethnicity, in pediatric patients presenting with unexplained neurodevelopmental delay and progressive microcephaly, associated or not with spastic tetraplegia or early onset epileptic encephalopathy. Abstract (versão em português): O papel do gene SLC1A4 no transporte de L-serina dos astrócitos para os neurónios (via sistema transportador dependente de Na + ASC) é um mecanismo fisiológico estabelecido. As variantes causadoras de doença no gene SLC1A4 foram identificadas como uma causa muito rara de distúrbios do desenvolvimento neurológico (tetraplegia espástica, corpo caloso fino e microcefalia progressiva - SPATCCM, OMIM 616657), relatadas principalmente em pacientes com antecedentes judaicos Ashkenazi (homozigotos para a variante p. (Glu256Lys)) (n = 15 dos 17 casos descritos). Neste artigo, relatamos dois casos de portugueses que apresentam atraso significativo no desenvolvimento global, microcefalia progressiva grave e espasticidade, apesar de não terem ascendência judaica Ashkenazi. O Whole exome sequencing identificou três novas variantes de SLC1A4 como a provável causa da doença. O paciente 1 é heterozigoto para duas variantes patogénicas no splice-site, c.527 + 1G> T e c.1035-1G> T. O impacto no sliping foi confirmado por um método de minigenes. O paciente 2 é homozigoto para uma variante missense provavelmente deletéria, c.272T> C (p. (Leu91Pro)), classificada como de significado clínico incerto. Nossos resultados apoiam o papel deletério das variantes do SLC1A4 e destacam a necessidade de considerar esta entidade, independentemente da etnia, em pacientes pediátricos que apresentem atraso inexplicável no neurodesenvolvimento e microcefalia progressiva, associada ou não a tetraplegia espástica ou encefalopatia epiléptica de início precoce. |
| Description: | Summary (English version): Neurodevelopmental childhood disorders present a wide spectrum of non-specific early onset abnormalities. The establishment of a molecular diagnosis is often challenging due to the complex pathophysiological networks involved. In this article, we made a review of the literature about the SLC1A4 variants causing disease described until now and add two new patients also carriers of SLC1A4 gene pathogenic variants and presenting with significant global developmental delay, severe progressive microcephaly and spasticity. Resumo (versão em português): As patologias pediátricas do neurodesenvolvimento apresentam um amplo espectro de alterações inespecíficas de início precoce. O estabelecimento de um diagnóstico molecular é muitas vezes desafiador devido às complexas redes fisiopatológicas envolvidas. Neste artigo, fizemos uma revisão da literatura sobre as variantes do SLC1A4 causadoras de doença descritas até agora, e adicionamos dois novos pacientes também portadores das variantes patogénicas do gene SLC1A4 que apresentam um atraso significativo no desenvolvimento global, microcefalia progressiva grave e espasticidade. |
