dc.descriptionObjetivos: O ensaio PARADIGM-HF estabeleceu a superioridade do sacubitril/valsartan (Sac/Val) sobre a inibição da Enzima Conversora da Angiotensina no tratamento da Insuficiência Cardíaca com Fração de Ejeção Reduzida (IC-FER). Neste estudo, caracterizamos os desafios da implementação do sacubitril/valsartan na prática clínica real. Métodos e resultados: Analisamos os registos clínicos dos doentes com IC-FER seguidos na clínica de Insuficiência Cardíaca e Transplante de um hospital terciário português em 2018 (n=240). Foram quantificados o número de candidatos a Sac/Val, o número de doentes a quem foi efetivamente prescrito o fármaco e as razões para não prescrição de Sac/Val. Adicionalmente, estudamos a tolerabilidade de doses máximas de Sac/Val. A classe functional da New York Heart Association (NYHA) e a Fração de Ejeção do Ventrículo Esquerdo (FEVE) antes e depois de atingir a dose tolerada máxima de Sac/Val foram comparadas. Entre 168 candidatos a Sac/Val, apenas a 91 (54,2%) foi prescrito o fármaco. As duas principais razões para a não prescrição foram barreiras económicas por parte do doente (31,2%) e hipotensão (27,3%). Apenas 34,4% atingiram dose máxima de Sac/Val. A causa mais frequente para sub-titulação foi hipotensão. A duração do tratamento com Sac/Val mostrou uma associação positiva com a FEVE (6,91% de aumento absoluto de FEVE/ano). Observou-se ainda uma melhoria significativa da classe funcional de NYHA entre o início da toma de Sac/Val e o fim do follow-up. Conclusão: Em condições de prática clínica real, o sacubitril/valsartan mostrou diferenças positivas na classe da NYHA e na FEVE. Contudo, há importantes obstáculos financeiros e clínicos à sua implementação. dc.description.abstractAims: The PARADIGM-HF trial established the superiority of sacubitril/valsartan (Sac/Val) over Angiotensin Converting Enzyme inhibition in Heart Failure with reduced Ejection Fraction (HFrEF). In the present study, we aimed to characterise the challenges of sacubitril/valsartan use in everyday clinical conditions. Methods and results: We assessed the medical records of HFrEF patients attending a HF clinic at a tertiary Portuguese hospital during 2018 (n=240). The number of Sac/Val candidates, the number of patients who effectively received the drug, and the reasons for not prescribing Sac/Val were quantified. Additionally, we assessed the tolerability of maximal doses of Sac/Val. New York Heart Association functional class (NYHA class) and Left Ventricular Ejection Fraction (LVEF) before and after maximum tolerated Sac/Val dose were compared. Out of the 168 Sac/Val candidates, only 91 (54.2%) were prescribed the drug. The two main reasons for non-prescription were patient financial barriers (31.2%) and hypotension (27.3%). Only 34.4% reached Sac/Val maximal dose. Hypotension was the most frequent reason for under-titration. Duration of Sac/Val treatment showed a positive association with LVEF (6.91% absolute increase in LVEF/year). NYHA functional class significantly improved from baseline through the end of follow-up. Conclusions: Sacubitril/valsartan showed a positive change in NYHA functional class and in LVEF under real-life conditions. However, important financial and clinical barriers to the implementation of this therapy were identified.
