| Title: | Neonatal Polycythemia: Prevalence, Risk Factors and Predictors of Severity |

| Abstract: | Background: Polycythemia is a hematological condition with several causes and risk factors. The clinical presentation is variable, ranging from asymptomatic newborns to cases with severe physiological changes that can be fatal. The aim of this study was to assess the prevalence, risk factors and predictors of severity of polycythemia in a Portuguese level III Neonatal Intensive Care Unit (NICU). Methods: Retrospective study of all term newborns admitted to the NICU of "Centro Hospitalar e Universitário de São João" between 1999 and 2019 and meeting one of the following inclusion criteria were eligible for the study: 1) Hematocrit > 65% or Hemoglobin > 22 g/dl; 2) Hemoglobin ≥ 21 g/dl with clinical manifestations of polycythemia. Results: Of the 78 newborns diagnosed with polycythemia, a total of 53 fulfilled the inclusion criteria and were included in the study, corresponding to a prevalence of 0.57%. Birth outside the hospital was the only risk factor with statistical significance. Of 53 cases, 51 (96.23%) had symptomatic polycythemia. The most frequent symptoms were: hyperbilirubinemia (69.81%), hypoglycemia (52.83%), thrombocytopenia (50.94%), cardiorespiratory (33.96%) and neurological symptoms (33.96%). Of the 53 newborns evaluated, 41 (77.36%) needed treatment. The only risk factors that influenced the hematocrit value were maternal diabetes and fetal growth restriction. Conclusions: The best way to improve the prognosis of polycythemia is to identify the risk factors present throughout pregnancy and make an early diagnosis and treatment. Out-of-hospital births should be avoided. The diagnosis shouldn't be excluded, even if hemoglobin and hematocrit are within normal limits. |
| Description: | Enquadramento: A policitemia é uma patologia hematológica com várias causas e fatores de risco. A apresentação clínica é variável, indo desde recém-nascidos assintomáticos a casos com graves alterações fisiológicas que podem levar à morte. O objetivo deste estudo foi avaliar a prevalência, fatores de risco e determinantes de gravidade de policitemia numa Unidade de Cuidados Intensivos Neonatais (UCIN) portuguesa de nível III. Métodos: Estudo retrospetivo de todos os recém-nascidos de termo admitidos na UCIN do Centro Hospitalar e Universitário de São João entre 1999 e 2019 e que preenchiam um dos seguintes critérios de inclusão foram elegíveis para o estudo: 1) Hematócrito > 65% ou Hemoglobina > 22 g / dl ; 2) Hemoglobina ≥ 21 g / dl com manifestações clínicas de policitemia. Resultados: Dos 78 recém-nascidos com policitemia, um total de 53 preencheram os critérios de inclusão e foram incluídos no estudo, correspondendo a uma prevalência de 0,57%. O nascimento fora do hospital foi o único fator de risco com significância estatística. Dos 53 casos, 51 (96,23%) apresentaram policitemia sintomática. Os sintomas mais frequentes foram: hiperbilirrubinemia (69,81%), hipoglicemia (52,83%), trombocitopenia (50,94%), sintomas cardiorrespiratórios (33,96%) e neurológicos (33,96%). Dos 53 recém-nascidos avaliados, 41 (77,36%) necessitaram de tratamento. Os únicos fatores de risco que influenciaram o valor do hematócrito foram a diabetes materna e a restrição de crescimento fetal. Conclusão: A melhor forma de melhorar o prognóstico da policitemia é identificar os fatores de risco presentes durante a gravidez e fazer um diagnóstico e tratamento precoces. Partos extra-hospitalares devem ser evitados. O diagnóstico não deve ser excluído, mesmo que os valores de hemoglobina e de hematócrito estejam dentro dos limites da normalidade. |
