dc.descriptionFGF23 é uma importante hormona reguladora do metabolismo do fosfato, conhecida principalmente pelos seus efeitos fosfatúricos e pela sua associação com a doença renal crónica. Recentemente, o papel do FGF23 na patofisiologia das doenças cardiovasculares tem recebido maior atenção. Mais especificamente, vários autores têm fornecido dados epidemiológicos no que diz respeito à associação entre níveis plasmáticos elevados de FGF23 e o risco de doença aterosclerótica. No entanto, os mecanismos subjacentes a esta relação permanecem pouco claros, apesar de alguns estudos mostrarem evidência de ações específicas do FGF23 no sistema cardiovascular. Embora os seus efeitos hipertróficos sobre o miocárdio estejam bem descritos, os efeitos sobre a vasculatura estão muito menos clarificados. Considerando o modo como o eixo FGF23-klotho pode afetar os componentes estrutural (calcificação vascular) e funcional (disfunção endotelial) da parede vascular, parece que o FGF23 apresenta um efeito anti-calcificação mas, por outro lado, foi encontrada associação com compromisso da vasoreatividade, sobretudo com declínio da vasodilatação periférica dependente do endotélio, o que aponta, indiretamente, para potencial lesão endotelial. O objetivo do presente artigo consiste em rever os principais mecanismos subjacentes aos efeitos do FGF23 na vasculatura, em condições tanto fisiológicas como patofisiológicas.
dc.description.abstractFGF23 is a major phosphate regulatory hormone mostly knonw by its phosphaturic effects and its association with chronic kidney disease. More recently, FGF23 role in cardiovascular disease pathophysiology has received increasing attention. More specifically, several authors have provided epidemiologic data regarding the association between higher plasma levels of FGF23 and the risk of atherosclerotic disease. However, the mechanisms underlying this relationship remain unclear although some studies have shown evidence of FGF23 specific actions within the cardiovascular system. Though its hypertrophic myocardial actions have been well described, FGF23 effects within the vasculature appear much less clear. Considering how FGF23-Klotho axis might affect both structural (vascular calcification) and functional (endothelial dysfunction) components of the vascular wall, it appears that FGF23 has an anticalcific effect but, on the other hand, it was found to be associated with impaired vasoreactivity mostly by decreasing peripheral endothelium-dependent vasodilatation, indirectly pointing to endothelium injury. The objective of the present work is to review the main mechanisms underlying FGF23 effects within the vasculature both in physiologic and pathophysiologic conditions.
