| Title: | The role of Ketamine and Enantiomers in depression: comprehensive clinical review |

| Abstract: | Major Depressive Disorder (MDD) is a common, distressful and disabling mental disorder. The currently available therapeutic agents have important limitations, as treatment-resistant depression (TRD) is experienced by 12-20% up to a third of patients. (R,S)-ketamine is the aryl-cyclo-hexylamine derivative synthetized from phencyclidine (PCP), composed by a mixture 1:1 of the (R) and (S) enantiomers. Clinical trials of (R,S)-ketamine, in all delivery formulations, and recently intranasal (S)-ketamine, showed good efficacy and safety profiles, though (R)-ketamine in preclinical trials regards great promise towards a better profile. This review covers the latest developments of ketamine (and it´s enantiomers) as a new-class antidepressant, such as clinical efficacy, safety and the role of electroconvulsive therapy (ECT). It also points the current limitations and directions towards the applicability of ketamine in routine clinical practice. |
| Description: | A Perturbação Depressiva Major (PDM) é uma doença mental frequente, angustiante e incapacitante. As opções terapêuticas atualmente disponíveis têm limitações importantes, com a depressão refratária ao tratamento a ser experienciada por 12 a 20% até um terço dos pacientes. A (R,S)-cetamina é um derivado da aril-ciclo-hexilamina sintetizado a partir da fenciclidina, composto por uma mistura 1:1 de enantiómeros (R) e (S). Ensaios clínicos da (R,S)-cetamina, em todas as vias de administração, e recentemente da (S)-cetamina, demonstraram bons perfis de eficácia e segurança, embora a (R)-cetamina, em ensaios pré-clínicos, apresente uma grande promessa relativamente a um perfil melhor. Esta revisão aborda os desenvolvimentos mais recentes da cetamina enquanto nova classe de antidepressivos, tal como eficácia clínica, segurança e o papel da eletroconvulsoterapia. São, também, apontadas as principais limitações e orientações relativas à aplicabilidade da cetamina na prática clínica. |
