| Title: | Depression and anxiety in juvenile systemic lupus erythematosus - kidney involvement as a specific biological signature |

| Abstract: | When systemic lupus erythematosus strikes children younger than 18 years of age, it is known as Juvenile systemic lupus erythematosus (JSLE). Depression and anxiety are important aspects of lupus, but these psychological manifestations are not well understood in JSLE. To better understand the clinical and psychological factors associated with depression and anxiety in JSLE, we investigated these in a cohort of JSLE patients. The patients were compared to adult-onset SLE patients, depressed patients, and normal controls. Methods: Twenty-nine JSLE patients were recruited for the study. They were given a variety of surveys to test their psychological status and perceptions about their health, including the Hospital Anxiety and Depression scale, the Fatigue Severity Scale, and a Medical outcomes questionnaire (the SF-36). Multiple clinical lab test results were obtained from the patients' medical records. Univariate and Multivariate analyses were used to compare the patient populations and identify correlations between key physical and psychological indicators. Results: The most striking finding was that kidney disease was highly associated with depression in the JSLE patients. The BUN levels, BUN/creatinine ratio, and leukocyturia were all significantly associated with depressive symptoms. The BUN/creatinine ratio was the most predictive laboratory value for both depression and anxiety. The JSLE patients showed significantly less depression overall than the adult-onset SLE patients, although anxiety was not significantly different. There was also no significant difference between the JSLE patients and the SLE patients in the laboratory tests. JSLE patients had comparable levels of fatigue and pain to the adult-onset SLE patients. Psychosocial factors that correlated with depressive symptoms in the JSLE patients included fatigue, pain, poor general health, and inability to perform social and physical roles. The primary psychosocial factors that correlated with anxiety were decreased perception of social and emotional function. Conclusions: Kidney involvement is usually an indicator of severe lupus, and better correlated with symptoms of depression and anxiety than any other clinical measures. This may mean that symptomatology is an important indicator of whether the patient could need psychiatric care. Kidney involvement is especially important in JSLE, where it is more common than in adult-onset SLE. The JSLE patients were more anxious than depressed. The decreased overall depression among JSLE patients, despite similar clinical and psychosocial values, bears further investigation. |
| Description: | Quando o Lupus Eritematoso Sistémico (LES) atinge jovens com menos de 18 anos, é conhecido como Lupus Eritematoso Sistémico Juvenil (LESJ). Depressão e ansiedade são aspetos importantes do lúpus, mas estas manifestações psicológicas não estão bem estudadas no LESJ. Para melhor compreender os fatores clínicos e psicológicos associados à depressão e ansiedade no LESJ, investigámos estes fatores numa coorte de pacientes LESJ. Os pacientes foram comparados aos pacientes com LES adulto, pacientes deprimidos e controlos normais. Métodos: Vinte e nove pacientes LESJ foram recrutados para o estudo. Foi-lhes administrada uma série de questionários para testar o seu estado psicológico e perceções sobre a sua saúde, incluindo o Hospital Anxiety and Depression scale, o Fatigue Severity Scale, e o Medical outcomes questionnaire (the SF-36). Múltiplos resultados de testes laboratoriais foram obtidos dos registos clínicos dos doentes. Análises univariadas e multivariadas foram usadas para comparar as populações de doentes e identificar correlações entre indicadores físicos e psicológicos importantes. Resultados: O achado mais surpreendente foi que a doença renal estava associada à depressão nos pacientes LESJ. Os níveis de BUN, rácio BUN/Creatinina e leucocitúria estavam todos significativamente associados a sintomas depressivos. O rácio BUN/Creatinina foi o valor laboratorial com melhor valor preditivo para ambas a depressão e ansiedade. Os pacientes LESJ mostraram valores significativamente mais baixos de depressão do que os pacientes LES adultos, apesar dos valores da ansiedade não mostrarem diferenças significativas. Também não se encontrou diferença significativa nos testes laboratoriais entre os pacientes LESJ e os LES adultos. Os pacientes LESJ tinham valores de fadiga e dor comparáveis com os dos pacientes LES adultos. Fatores psicossociais que correlacionavam com sintomas depressivos nos pacientes LESJ incluíam fadiga, dor, mau estado de saúde geral e incapacidade de cumprir papéis sociais e físicos. Os fatores psicossociais primários que correlacionavam com a ansiedade foram a perceção diminuída da função social e emocional. Conclusões: Envolvimento renal é geralmente um indicador de lúpus severo, e mais correlacionado com sintomas de depressão e ansiedade do que outras medidas clínicas. Isto pode significar que sintomatologia é um indicador importante da necessidade de cuidados psiquiátricos. Envolvimento renal é especialmente importante no LESJ, pois é mais comum do que no LES adulto. Os pacientes LESJ eram mais ansiosos do que deprimidos. Este valor mais baixo de depressão nos pacientes LESJ, apesar de terem valores clínicos e psicossociais semelhantes, necessita de mais investigação. |
