Title: Management of the carotid artery stenosis in asymptomatic patients

Abstract: Background: Moderate/severe carotid artery stenosis (CS) has a prevalence of 0.3 to 4.5% in women and 0.5% to 5.7% in men. The bifurcation of the common carotid artery turns this area vulnerable to atherosclerosis due to the features of haemodynamic flow. The exact prevalence of asymptomatic patients with CS remains to be unknown and there is a controversial opinion for the treatment of this patients. Objective: The authors aimed to review the evidence on the management of the asymptomatic CS and describe its clinical characteristics, diagnosis and treatment management. Results: The majority of patients with asymptomatic CS should be submitted to conservative management with best medical therapy. However, selective surgical management should be considered if high risk features are present. Conclusion: Medical intervention has an established role in the current routine management of persons with CS. Only extensive information allows tailored evidence-based decision.
Description: Background: A estenose moderada/grave da artéria carótida tem uma prevalência de 0,3 a 4,5% em mulheres e 0,5 a 5,7% em homens. A bifurcação da artéria carótida comum torna essa área vulnerável à aterosclerose devido às características do fluxo hemodinâmico. A prevalência exata de pacientes assintomáticos com estenose carotídea ainda é desconhecida e existe uma opinião controversa em relação ao tratamento destes doentes. Objectivo: Os autores tiveram como objetivo rever as evidências sobre a intervenção na estenose carotídea assintomática e descrever as suas características clínicas, o seu diagnóstico e tratamento. Resultados: A maioria dos pacientes com estenose carotídea assintomática deve ser submetida a tratamento conservador com a melhor terapia médica. No entanto, o tratamento cirúrgico seletivo deve ser considerado se estiverem presentes características de alto risco. Conclusão: A intervenção médica tem um papel estabelecido na atual terapêutica de pacientes com estenose carotídea. Somente o estudo detalhado imagiológico e clínico permitem uma decisão informada.
