dc.descriptionA pandemia de COVID-19 reduziu os níveis de atividade física e aumentou a incidência de estilos de vida sedentários em resultado das medidas de confinamento a nível mundial. Esta situação afetou tanto as pessoas saudáveis como as que sofrem de doenças crónicas não transmissíveis. No entanto, as pessoas que sofrem de doenças em que a mobilidade é afetada, como a osteoartrite (OA), podem ter sido ainda mais afetadas por esta situação. Esta tese investigou os níveis de atividade física no contexto chileno, considerando variáveis sociodemográficas e caraterísticas de saúde em pessoas que sofrem de OA, comparando indivíduos saudáveis com aqueles que sofrem desta patologia. Foram realizados quatro estudos. O primeiro deles (primeiro artigo) procurou analisar se existia uma relação entre as variáveis sociodemográficas (sexo, idade, estado civil, nível de escolaridade, nível socioeconómico e modo de trabalho) e os níveis de atividade física (vigorosa, moderada e caminhada) reportados por adultos chilenos pós-confinamento devido à pandemia da COVID-19. Dos quatrocentos e oitenta e seis participantes, 84 adultos mais idosos foram recrutados no segundo artigo, dos quais 42 tinham OA do joelho, anca ou ambas e os restantes não tinham diagnóstico da doença, a fim de comparar o nível de atividade física e a qualidade de vida entre pacientes com OA e sujeitos saudáveis. Alguns destes sujeitos fizeram ainda parte do terceiro e quarto artigos, perfazendo 80 participantes no total com o objetivo de examinar se os antecedentes de saúde, a atividade física e o comportamento sedentário explicam variações na intensidade dos sintomas de osteoartrite de joelho e anca. Foram utilizados questionários e solicitada informação dos registos médicos para avaliar os níveis de atividade, o grau de gravidade, o diagnóstico de OA do joelho, da anca ou de ambos e se era bilateral ou não, bem como a sintomatologia das pessoas com OA em termos de função física, rigidez e dor. Os resultados sugerem que os homens, as pessoas mais jovens, as que não coabitam com um parceiro, as que têm um estatuto socioeconómico mais baixo e as que trabalham presencialmente praticam níveis mais elevados de atividade física. Ao comparar os níveis de atividade física entre idosos com e sem OA, não foram encontradas diferenças. No entanto, as pessoas com OA relataram uma melhor qualidade de vida do que as pessoas com OA. Por sua vez, as pessoas com OA referiram que a sua qualidade de vida era melhor quando caminhavam mais. Ao analisar especificamente a relação entre o historial de saúde dos doentes com OA da anca e do joelho e os seus sintomas com os níveis de atividade física no terceiro e quarto estudos, verificou-se que a funcionalidade física era explicada pelo envolvimento bilateral, pelo nível de dor e pela gravidade da OA quando tinham níveis mais baixos de atividade física. Especificamente, níveis mais elevados de atividade física intensa, moderada e de caminhada, juntamente com níveis mais baixos de atividade sedentária, mediaram a relação entre a gravidade da doença, a bilateralidade e o tipo de OA com os níveis de dor e a função física. A rigidez foi explicada principalmente pela gravidade da doença, mediada pela atividade física e pelo comportamento sedentário. É necessária mais investigação para explorar as interações complexas entre a história da doença, a atividade física e os resultados de saúde em diferentes populações, embora os resultados da investigação que compôs esta tese de doutoramento contribuam para o estado atual do conhecimento sobre a promoção da atividade física para reduzir o comportamento sedentário na população chilena com e sem OA no período pós-pandémico. dc.description.abstractThe COVID-19 pandemic reduced physical activity levels and increased sedentary lifestyles due to worldwide confinement measures. This situation affected both healthy people and people suffering from chronic non-communicable diseases. However, people suffering from diseases in which mobility is affected, such as Osteoarthritis (OA), could be even more adversely affected by this situation. This thesis investigated physical activity levels in the Chilean context, considering sociodemographic variables and health characteristics in people suffering from OA and comparing healthy subjects with those suffering from this pathology. Four studies were carried out. The first of these articles sought to analyze whether there was a relationship between sociodemographic variables (sex, age, marital status, educational level, socioeconomic level, and type of work) and levels of physical activity (vigorous, moderate, and walking) reported by Chilean adults after the COVID-19 pandemic. Of the four hundred and eighty-six participants, 84 older adults were recruited in the second article, of which 42 had OA of the knee, hip, or both, and the rest were not diagnosed with the disease to compare the level of physical activity and quality of life. Some were part of the third and fourth articles, leaving 80 participants in total to examine whether health antecedents, physical activity, and sedentary behavior explain variations in the intensity of knee and hip osteoarthritis symptoms. Questionnaires were used, and information described in medical records was requested to assess activity levels, degree of severity, diagnosis of OA of the knee, hip, or both and whether it was bilateral or not, as well as the symptomatology of people with OA in terms of physical function, stiffness, and pain. The results suggest that men, younger people, those not cohabiting with a partner, those of lower socioeconomic status, and those working face-to-face performed higher physical activity. No differences were found when comparing physical activity levels between older adults with OA and those without. However, people with OA reported a better quality of life than those with OA. In turn, those with OA reported that their quality of life was better when they walked more. When specifically analyzing the relationship between the health history of hip and knee OA sufferers and their symptoms with physical activity levels in the third and fourth studies, it was found that physical functionality was explained by bilateral involvement, pain level, and OA severity when they performed lower levels of physical activity. Specifically, higher levels of intense, moderate, and walking physical activity and lower levels of sedentary activity mediated the relationship between disease severity, bilaterality, and OA type with pain levels and physical function. On the other hand, stiffness was mainly explained by disease severity, mediated by physical activity and sedentary behavior. Further research is needed to explore the complex interactions between disease history, physical activity, and health outcomes in different populations, although the results of the research that composed this doctoral thesis contribute to the literature on the promotion of physical activity to reduce sedentary behavior in the Chilean population with or without OA in the post-pandemic period.
