Title: Physical and mental health benefits of physical exercise in persons with schizophrenia: The contribution of outdoor physical activities

Abstract: intersection of physical and psychological factors, along with psychiatric symptoms and medication, influences both the quality of life and mood states of persons with schizophrenia. In the last decade, there has been growing recognition of the importance of active lifestyles in persons with schizophrenia but with little emphasis on mood states. Moreover, physical activity programs have been shown to improve both mental and physical health. Specifically, nature-based exercise in urban settings is an innovative and trendy intervention with a potentially promising effect in health but lacking evidences in the population with schizophrenia. In addition, the lack of validated instruments to assess mood states in persons with schizophrenia highlights the need to validate a specific tool. Thus, the main objective of this doctoral thesis was to understand the contribution of outdoor exercise in relation to physical and mental benefits in persons with schizophrenia. The doctoral thesis included five studies that aimed: i) to assess the construct validity of the Portuguese short version of the Profile of Mood States (POMS-SV) for persons with schizophrenia and ii) to conduct a comparative analyse of mood states according to different demographic variables (study 1); iii) to assess the association between physical health-related factors and quality of life on mood states in a sample of persons with schizophrenia (study 2); iv) to describe the protocol needed to evaluate the feasibility and effectiveness of a 24-week outdoor exercise program and 12-week follow-up on the physical and mental health of persons with schizophrenia (study 3); iv) to assess the feasibility and effectiveness of a 24- weeks outdoor exercise program in physical and mental health of persons with schizophrenia (study 4); and v) to examine the long-term mental and physical health benefits of a 24-week outdoor exercise program after a 12-week follow-up (study 5). A total of 106 participants were included in the thesis with different sub-samples for the different studies. Standardised assessment instruments (i.e., questionnaires and field test) were applied to assess mental and physical determinants. The results of the studies showed that: i) the Portuguese version of the POMS-SV proved to be a valid and reliable assessment tool for persons with schizophrenia and significant differences were found in gender, marital status, and engaged PA in the negative mood sub-scales; ii) quality of life, gender, years of illness and physical activity engagement are significant predictors of mood states in both in- and outpatients with schizophrenia; iii) a 24-week outdoor exercise program significantly improves functional exercise capacity and balance and reduces weight and body mass index; iv) after 12-week follow-up functional exercise capacity improvements were lost, flexibility worsened and participants reported significant improvements in quality of life - physical domain. This doctoral thesis has presented new and pertinent evidence on outdoor exercise influencing mental and physical health of persons with schizophrenia. Several practical recommendations can be emphasised to guide integrated active behaviours interventions in the schizophrenia treatment. Description: A intersecção de fatores físicos e psicológicos, juntamente com os sintomas psiquiátricos e a medicação, influencia tanto a qualidade de vida como os estados de humor das pessoas com esquizofrenia. Na última década, tem havido um reconhecimento crescente da importância de estilos de vida ativos em pessoas com esquizofrenia, mas com pouca ênfase nos estados de humor. Além disso, tem sido demonstrado que os programas de atividade física melhoram a saúde física e mental. Especificamente, o exercício realizado em contacto com a natureza e em contextos urbanos é uma intervenção inovadora e atual com um efeito potencialmente promissor na saúde, mas que carece de evidências na população com esquizofrenia. Para além disso, a falta de instrumentos validados para avaliar os estados de humor em pessoas com esquizofrenia realça a necessidade de validar uma ferramenta de avaliação específica. Assim, o principal objetivo desta tese de doutoramento foi compreender o contributo do exercício físico ao ar livre em relação aos benefícios físicos e mentais em pessoas com esquizofrenia. A tese de doutoramento incluiu cinco estudos que tiveram como objetivos i) avaliar a validade de constructo da versão curta portuguesa do Profile of Mood States (POMS-SV) para pessoas com esquizofrenia e ii) realizar uma análise comparativa dos estados de humor de acordo com diferentes variáveis demográficas (estudo 1); iii) avaliar a associação entre fatores relacionados com a saúde física e a qualidade de vida nos estados de humor numa amostra de pessoas com esquizofrenia (estudo 2); iv) descrever o protocolo necessário para avaliar a viabilidade e a eficácia de um programa de exercício físico ao ar livre de 24 semanas e um acompanhamento de 12 semanas na saúde física e mental de pessoas com esquizofrenia (estudo 3); v) avaliar a aplicabilidade e a eficácia de um programa de exercício ao ar livre de 24 semanas na saúde física e mental de pessoas com esquizofrenia (estudo 4); e vi) examinar os benefícios a longo prazo para a saúde física e mental de um programa de exercício ao ar livre de 24 semanas após um acompanhamento de 12 semanas (estudo 5). A tese incluiu um total de 106 participantes, com diferentes subamostras para os diferentes estudos. Foram aplicados instrumentos de avaliação normalizados (ou seja, questionários e testes de campo) para avaliar as determinantes mentais e físicas. Os resultados dos estudos mostraram que: i) a versão portuguesa do POMS-SV provou ser um instrumento de avaliação válido e fiável para pessoas com esquizofrenia e foram encontradas diferenças significativas no género, estado civil e prática de AF nas subescalas de humor negativo; ii) a qualidade de vida, o género, os anos de doença e a prática de atividade física são preditores significativos dos estados de humor em pessoas com esquizofrenia, tanto em regime de internamento como em ambulatório; iii) XIV um programa de exercício ao ar livre de 24 semanas melhora significativamente a capacidade funcional para o exercício l e o equilíbrio, bem como reduz o peso e o índice de massa corporal; iv) após um acompanhamento de 12 semanas, as melhorias na capacidade de exercício funcional perderam-se, a flexibilidade piorou significativamente e os participantes relataram melhorias significativas na qualidade de vida - domínio físico. Esta tese de doutoramento apresentou novas e pertinentes evidências sobre a influência do exercício ao ar livre na saúde física e mental de indivíduos com esquizofrenia. Podem ser salientadas várias recomendações práticas para orientar intervenções integradas de comportamentos ativos no tratamento da esquizofrenia.
