Frailty phenotype in heart failure: A condition that transcends age

Title: Frailty phenotype in heart failure: A condition that transcends age
Abstract: Introduction and objectives: Studies on younger frail and pre-frail subjects suffering from heart failure (HF) are scarce, except for those focusing on the critically ill. This work aims to describe differences between younger (<65 years) and older (>= 65 years) pre-frail and frail HF outpatients regarding their nutritional, functional and clinical statuses. Methods: In this cross-sectional study, a sample of 99 HF frail and pre-frail patients (aged 24-81 years, 38.4% women, 21.2% frail, 59.6% <65 years) was recruited from an HF outpatients' clinic in northern Portugal. Muscle mass was estimated from mid-upper arm muscle circumference. Weight status was assessed using body mass index. Hand grip strength and gait speed were measured. Medical records were reviewed. Associations between participants' characteristics and age were calculated using binary logistic regression. Results: Age was associated with hand grip strength (OR=0.90), gait speed (OR=0.01) and dia-betes (OR=4.95). Obesity, muscle mass or heart failure functional classes were not associated with age categories. Conclusion: There is an overall lack of differentiation between younger and older HF patients with the frailty phenotype. Therefore, frailty phenotype should be assessed in all patients, regardless of age. Hand grip strength seems to be a good predictor for older age and more studies are needed to define age-specific hand grip strength cut-offs for HF populations.
NUTLY: um instrumento baseado em fotografias para medição da literacia nutricional

Title: NUTLY: um instrumento baseado em fotografias para medição da literacia nutricional
Abstract:

Abstract: This article analyses the experiences and perceptions of a sample of people over 60 about the most
frequent type of violence in old age, the risk factors, the offenders and the reactions to abuse. A mixed study was
carried out with the sequential application of 677 questionnaires and 45 interviews. The results show that older
people are mainly victims of psychological abuse and financial exploitation; the type of abuse varies according to
the type of aggressor; and that machismo, ageism, mental health problems, addictive behaviours and social
isolation are risk factors for violence. Health and justice professionals are perceived as those to whom victims
should turn and confide.

MONITORIA: The start of a new era of ambulatory heart failure monitoring? Part II - Design

Title: MONITORIA: The start of a new era of ambulatory heart failure monitoring? Part II - Design
Abstract: Introduction: Heart failure (HF) represents a huge financial and economic burden worldwide. Some authors advocate that remote monitoring should be implemented to improve HF management, but given its increasing incidence, as well as its morbidity and mortality, a question still remains: are we monitoring it properly? There is no shortage of literature on home monitoring devices, however, most of them are designed to monitor an unsuitable array of variables and, to the best of our knowledge, there are no large randomized studies about their impact on morbidity/mortality of HF patients. Objective: Description of a novel monitoring device. Methods: As a solution, we designed MONITORIA (MOnitoring NonInvasively To Overcome mortality Rates of heart Insufficiency on Ambulatory). Results: This is a multimodal device that will provide real time monitoring of vital, electrophysiological, hemodynamic and chemical signs, transthoracic impedance, and physical activity levels. The device is meant to perform continuous analysis and transmission of all data. Significant alterations in a patient's variable will alert the attending physician and, in case of potentially life-threatening situations, the national emergency medical system. The MONITORIA device will, also, have a function that sends shocks or functions as a pacemaker to treat certain arrhythmias/blockades. This function can be activated the very first time the patient utilizes it, based on their risk of sudden cardiac death. Discussion/Conclusions: MONITORIA is a promising device mostly because it is included in a follow-up program that takes into account a multi-perspective feature of HF development and is based on the real world patient, adapting innovations not to the disease but rather to the patients.
Title: Segmentação automática de imagens doppler transversais modo-b da bifurcação carotídea / Automatic segmentation in transverse ultrasound bmode images of the carotid artery
Abstract: Ultrasound imaging is extremely important for the assessment of disease severity in the carotid
arteries. The high complexity of irregular structures in transverse ultrasound images of abnormal
bifurcation of the common carotid artery brings tremendous difficulty to segmentation and
identification of vulnerable atherosclerotic plaques. Automatic algorithms for image segmentation
provide results minimizing the variability caused by subjective decisions and different operators. The
proposed algorithm for automatic segmentation of B-mode transverse images of the carotid
bifurcation facilitates structure selection for further analyses and provides an evaluation performance
of the methodology and a means for comparing different algorithm settings. Patient-specific Doppler
imaging is used to illustrate the new developed methodology.

Title: A percepção do belo e a proporção divina
Abstract: The aesthetic knowledge concerning the face one is widely important for the professionals of the Dentistry area and, moreover, it is of interest of the people of a general form. It has a trend, while professional, of if imposing the aesthetic values when really can be to observe the values of the proper individual. Many are the studies that present, of clear form, the differences between the individuals, as patient or people independent, how to its preferences, if compared with the ideas of beauty of the professionals of different areas. Being thus, objective in this chapter to integrate the reader to the presentation of the aesthetic one, not only in the scientific aspect, as measurable and quantitative object, as also in the artistic and social aspect. How to the beauty, the perception that if has of the things in day-by-day is basically utilitarian and conceptual. It has a bigger trend in if to perceive the things by means of the concepts proceeding from the language of the one that really eat is capable to sensitize our directions. However, it fits to remember that the language classifies and appraises the feelings, but is extremely impotent to describe them. It must always be had in mind that is in the sensible one - and not in the conceptual one, in the intellectual - that inhabits the being of the aesthetic object.

Title: Análise de duas metodologias na aferição de infiltração por corante em apicoplastias com retrobturações
Abstract: Different methods have been used in studies on marginal
infiltration caused by staining dye with the purpose
of evaluating the efficiency of dental materials
employed in apicoplasties with retrograde root filling.
Apicoplasty consists of flattening the root apex eliminating
the need of root amputation, maintaining its correct
shape. The objective of the present study was to
evaluate the results of infiltration of rhodamine B 1%
in an experimental group, comparing the dye depth penetration
with the impregnation area in the same samples
that underwent apicoplasties with retrograde root
filling. The samples included 12 human upper canine
teeth endodontically treated and retro-obturated using
MTA. For sample analysis, the Image Tool® software was
used to perform computerized morphometry. Results
showed a low standard deviation (4.79) regarding area
measurement, and high standard deviation (15.05) in
terms of measurement of the dye depth penetration. The
Mann-Whitney test showed statistically significant differences
between the groups (p < 0.0001). The use of the
dye stained area, instead of dye depth penetration, proved
to be a better choice for measuring marginal infiltration
scores according to the methodological parameters
used in the present study.

Abstract: Chylothorax is a possible complication after thoracotomy. Treatment is initially conservative and includes drainage and correction of nutritional losses. Surgical treatment is reserved for cases not responsive to medical measures. The authors report on the clinical case of a premature neonate with a traumatic chylothorax secondary to esophageal atresia surgical correction. The use of octreotide allowed the resolution of the effusion without adverse effects and avoided the surgical treatment.
